Coaching Agreement

This Agreement is entered into between MindFit Coaching, LLC ("MindFit" or "the Company") and the individual enrolling in its programs or services ("Member"). Services are delivered by MindFit Coaching, LLC and any coach, facilitator, or contractor engaged on behalf of the Company (collectively, "Coach" or "Coaches"). By purchasing or accessing any MindFit program or service, the Member agrees to all terms below. Coaching is not therapy.

1. Scope of Services

MindFit Coaching, LLC provides online coaching services focused on personal growth, emotional intelligence, stress management, and professional development. Coaching is not a substitute for psychotherapy, medical treatment, or crisis intervention.

2. Coaching Relationship

Coaching is a collaborative, Member-driven process. The Member takes full responsibility for their decisions and actions. Coaches provide guidance, tools, and feedback to support Members in achieving their goals. MindFit Coaching, LLC reserves the right to assign, reassign, or substitute coaches at any time to ensure continuity of service.

3. Confidentiality

All information shared during coaching sessions is confidential and will not be disclosed to third parties without the Member's consent, except as required by law (see Section 4). Digital communications are conducted via secure platforms, but absolute confidentiality cannot be guaranteed due to the nature of online interactions.

4. Mandated Reporting

Some MindFit Coaches are licensed mental health professionals and mandated reporters under Florida law. Disclosure may be required in the following circumstances:

• Suspected abuse or neglect of a child, elder, or dependent adult.

• Credible threats of harm to self or others.

• A valid court order requiring disclosure.

• Where disclosure is required, the Member will be informed to the extent permitted by law.

5. Limits of Coaching

Coaching is distinct from therapy. If a Member's needs exceed the scope of coaching at any time, the Coach will recommend referral to appropriate licensed clinical or medical professionals.

6. Your Rights

As a Member, you have the right to:

• Ask questions at any time regarding program content or procedures.

• Discontinue participation at any time. (Please review Section 7 regarding our refund policy.)

• Be informed of decisions and actions that affect your participation.

• Request a list of referral sources outside the scope of MindFit coaching services.

7. Refund Policy

Coaching Programs (Registered Intern Roadmap & Beyond Labels): MindFit Coaching, LLC offers a 7-day money-back guarantee on all coaching program enrollments. If you are not satisfied for any reason, a full refund may be requested within 7 calendar days of the original purchase date by contacting [email protected]. Requests submitted after the 7-day window will not be honored. After 7 days, all sales are final.

Coaching Calls: All coaching call purchases — whether individual sessions or bundled packages — are final sale. No refunds will be issued under any circumstances, including unused sessions, late cancellations, or no-shows.

8. Payment Plans

Payment plans require separate installment payments due at predetermined intervals as outlined on the program's sales page. All payments must be completed to maintain program access. Failed or missed payments will result in immediate suspension of access until payment is successfully processed. MindFit Coaching, LLC reserves the right to pursue collection of any unpaid balances.

9. Program Length and Fees

Fees for all programs and services are due in full or according to the applicable payment plan. Payment is accepted via credit card only.

10. Coaching Calls

By purchasing any coaching call or package, you agree to the following:

General Terms (All Programs)

• An intake form must be completed prior to your first appointment.

• Rescheduling requests must be submitted no less than 24 hours prior to the scheduled appointment.

• Cancellations within 24 hours or no-shows will be forfeited without rescheduling or refund.

• Unused calls expire at the end of their applicable usage window and are non-refundable.

• MindFit Coaching, LLC reserves the right to assign any qualified Coach to deliver scheduled calls.

Registered Intern Roadmap

Call duration: 45 minutes.

Single Call: Must be scheduled and completed within 30 days of purchase.

Coaching Call Bundle (3 calls): All three calls must be completed within 90 days of purchase.

The Fourth Trimester

Call duration: 50 minutes.

Single Call: Must be scheduled and completed within 30 days of purchase.

Coaching Bundle (4 calls): All four calls must be completed within 60 days of purchase.

Deep Intensive (8 calls): All eight calls must be completed within 120 days of purchase.

Beyond Labels

Call duration: 50 minutes.

Single Call: Must be scheduled and completed within 30 days of purchase.

Coaching Bundle (4 calls): All four calls must be completed within 60 days of purchase.

Coaching Intensive (8 calls): All eight calls must be completed within 120 days of purchase.

11. Founder/Scholarship Promotions

From time to time, MindFit Coaching, LLC may offer "Founder" or "Scholarship" promotions providing program access at no cost via an exclusive promo code. These codes are non-transferable and intended solely for the recipient. In exchange, the Member agrees to:

Testimonial & Marketing Release: Provide a testimonial (written on the program's official Facebook page or via video) within 60 days of enrollment. By doing so, the Member grants MindFit Coaching, LLC a perpetual, irrevocable, royalty-free license to use their name, likeness, and testimonial for marketing and promotional purposes.

Privacy Disclaimer: Testimonials posted on social media platforms are public or semi-public in nature and fall outside the confidentiality protections in Section 3.

Feedback: Complete an internal program survey or provide detailed feedback within 60 days of enrollment.

Revocation of Access: Failure to provide both the testimonial and feedback within 60 days will result in immediate revocation of program access without further notice.

12. Consent to Participate

By purchasing any MindFit Coaching, LLC program or service, the Member affirms their understanding of the coaching process and agrees to all terms contained herein.

13. Coaching Consent

By purchasing, Members confirm they have read and understand this Agreement. Members acknowledge that:

• Coaching is not a substitute for psychotherapy, medical treatment, or crisis intervention.

• The coaching relationship is a non-medical, non-therapy relationship.

• They have the right to discontinue participation at any time and may request referral sources outside the scope of MindFit coaching.

• If they wish to pursue therapy, Coaches will provide referrals as appropriate.

• They consent to receive coaching services from MindFit Coaching, LLC and its designated Coaches.
